[摘要] 目的:
探讨多普勒超声检查对子宫内膜癌分期、黏膜层侵犯及淋
巴结转移诊断的价值,研究超声参数与微血管密度
及临床病理间的关系。
方法:
回顾性
分析80例经组织病理学证实的子宫内膜癌患者的临床资料,所有患者均接受超声、X
射线计算机断层扫
描(CT)及磁共振成像(MRI)检查,比较3种不同影像学检查对子宫内膜分期、黏膜层浸润及淋巴结转移的
诊断及鉴别诊断。
结果:
MRI对内膜癌分期、黏膜层侵犯及淋巴结转移诊断符合率最
高,CT与超声的术前分期结果、判断
黏膜层侵犯、淋巴结转移符合率比较差异无统计学意义;不同年龄
间、不同组织学类型间、是否发生淋巴结转移患者的超
声参数阻力指数(RI)、搏动指数(PI)及微
血管密度(MVD)水平的比较,其差异无统计学意义;不同病理分期、子宫肌层浸润
深度特征患者的R

结论:
MR
I、CT
及超声是子宫内膜癌诊断与疗效评估的常用影像学手段,虽然超声检查符合率不及MRI,但其
具有操作简单、检查方便等
优势,并且超声参数同样与子宫内膜癌微血管密度、临床病理间具有密切联系
,具有早期诊断筛查以及术前病情初步评估
的应用价值。

[Abstract]
Objective:
To explore the value of Doppler
ultrasonography in the diagnosis of endometrial
carcinoma
staging, mucosal invasion and lymph
node metastasis, and to study the relationship
between ultrasound parameters
and microvessel
density(MVD), clinical pathology.
Methods:
A retrospective analysis was made on the clinical
data
of 80 patients with endometrial carcinoma
who were confirmed by histopathology. All the 80
patients were examined
by ultrasound, computed
tomography(CT) and magnetic resonance
imaging(MRI). The diagnosis and differential
diagnosis ofendometrial carcinoma staging,
mucosal invasion and lymph node metastasis between
three different
imaging examinations were
compared.
Results:
MRI had the highest
diagnostic coincidence rate in staging, mucosal
invasion and lymph node metastasis of
endometrial carcinoma, but there was no
significant difference between CT
and
ultrasound in comparisons of coincidence rates of
preoperative staging, judging of mucosal invasion
and lymph
node metastasis. There were no
significant differences in resistance index (RI),
pulsatility index (PI) and MVD levels
between
patients with different ages, between patients
with different histological type and between
patients with and
without lymph node
metastasis. While there were significant
differences in RI, PI and MVD indexes among
patients
with different pathological stages
andcharacteristics of myometrial invasion
depth(

<0.05).
Conclusion:
MRI, CT and ultrasonography are the common
imaging methods for the diagnosis and assessment
of
endometrial carcinoma. Although the
coincidence rate of ultrasonography is lower than
that of MRI, it also has the
advantages of
simplicity of operation and convenience of
examination, and ultrasonographic parameters are
closely
relative with the microvessel density
and clinicopathology of endometrial carcinoma. And
it has application values of
early diagnostic
screening and preliminary assessment of
preoperative state of disease.
